Receive care in a respectful and non-discriminatory environment that values their privacy, dignity, culture, and personal beliefs.
Privacy and Confidentiality

Expect full protection of their personal and health information, in accordance with HIPAA regulations and our Privacy Policy.
Informed Consent

Receive clear, accurate, and complete information regarding their care, services provided, and potential risks to make informed decisions.
Participation in Care
Be involved in developing and reviewing their care plan, and refuse services or treatment to the extent permitted by law.
Access to Records

Request access to, review, or receive copies of their medical records in a timely manner.
Communication

Communicate openly with staff and ask questions regarding their care, billing, or policies.
Grievances and Complaints

Voice concerns or complaints about services received without fear of discrimination or retaliation, and receive a timely response.
Safe Environment

Receive services in a safe, secure, and comfortable environment
Patient Responsibilities
To help ensure the best possible care, we ask that patients:
Provide Accurate Information
Share complete and honest information about their medical history, symptoms, medications, and needs
Follow the Agreed Plan of Care
Collaborate with our nursing staff and follow the care plan unless otherwise discussed or declined.
Treat Staff with Respect

Treat all staff with courtesy and respect, just as they will be treated.
Provide a Safe Environment

Ensure the home environment is clean, safe, and free of hazards for care to be delivered appropriately.
Notify of Changes

Inform Grace and Glow Recovery of any changes in their health status, needs, or contact information.
Adhere to Policies

Comply with company policies, including those related to scheduling, cancellation, and payment.
Ask Questions

Ask questions when instructions or information are not understood, and seek clarification when needed.
Fulfill Financial Obligations

Make timely payments for services rendered according to agreed-upon terms.
